No engagement is created by an inquiry

The $10 good-faith exam offering is for established participating-practice relationships, with up to three delegated services per exam under the stated program. It is connected to clinical oversight and proprietary EMR documentation, not standalone exam sales to outside organizations. Confirm eligibility, clinical appropriateness and complete pricing in the applicable engagement.
Spa Shield participation and the stated physician exam-participation policy are business conditions, not Florida statutory minimums. They do not displace applicable evaluation, prescribing, supervision, facility or recordkeeping requirements.
Do not submit patient information
Do not submit patient identifiers, records, diagnoses, photographs or other sensitive clinical information through the professional inquiry form. If information is sent accidentally, contact the privacy contact without repeating it. Handling remains subject to applicable obligations; these terms do not waive duties that cannot lawfully be waived.
